Provides ongoing support services to eligible participants in supported employment jobs.

A Person Centered Employment Plan will be completed by a participant’s supported employment specialist and updated as needed as required in the Supports for Community Living Policy Manual.

A Supported Employment Long-Term Support Plan will be completed by a participant’s supported employment specialist and updated as needed as required in the Supports for Community Living Policy Manual.

An employment specialist will document progress by submitting notes regarding a participant’s job development activity, participant’s job acquisition or stabilization activity, and a participant’s long-term employment plan.
